Immunization Submission (Online Vaccination Reporting)

Welcome to the Online Vaccine Reporting site for Windsor-Essex County students. Immunization information is provided to the Windsor-Essex County Health Unit (WECHU) by parents when they register their children for child care or school in Windsor-Essex County. Submit this form at the time of initial registration and each time your child receives a vaccination. This information is used to monitor immunization status in the community and to protect your child if an outbreak of a vaccine-preventable disease occurs.

Children attending schools or day nurseries in Ontario are required to be immunized for certain diseases or have a valid exemption under the Immunization of School Pupils Act, R.S.O. 1990, and Ontario Regulation 645/90., and the Day Nurseries Act, R.S.O. 1990.

Use this information to submit immunization dates for your child. Once we receive this information we will update our immunization records accordingly. All fields marked with an ‘*’ are mandatory.

Reporting Your Child’s Immunization

If your child receives immunization(s) from your health care provider, it’s important you contact the Windsor-Essex County Health Unit (WECHU) to provide the up to date immunization information. Health care providers don’t contact the health unit when they provide immunization to patients.

The WECHU keeps immunization records for all elementary and high school students and for children attending day nurseries.

You can update your child’s record or provide your child’s immunization history to the Health Unit in one of the following ways:

  • Use the secure online form,
  • Fax a copy of your child’s immunization history to 519-258-7288,
  • Mail a photocopy of your child’s immunization history to VPD Program, Windsor-Essex County Health Unit, 1005 Ouellette Avenue, Windsor, ON, N9A 4J8,
  • Drop off a photocopy of your child’s immunization history to any one of the Health Unit office locations,
  • Call the Vaccine Preventable Disease Program at 519-258-2146 ext. 1222.
